1986-2014年中国和云南出生缺陷先天性心脏病的流行趋势
（摘要）目的 了解1986-2014年中国和云南先心病监测情况，为防治先天性心脏病提供科学依据。方法 采用全国妇幼卫生监测网和云南“三网”监测及年报表，用spss 15.0软件整理，运算，制图等，将2014年全国先天性心脏病省市发生率作系统聚类分析和作逐步回归分析，用PEM8 3.0软件作升降趋势检验（Cox stuart检验）
结果 1. 1996-2014年全国出生缺陷发生率逐年上升 2. 1989-2014年出生缺陷发生率呈波浪式下降，总体趋势随时间的推移而上升，云南出生缺陷发生率低于全国平均水平。 3. 出生缺陷类型及顺位，全国主要出生缺陷发生率第一位病种，除2000年为总唇裂外，从2005-2014年均为先天性心脏病. 4. 2014年全国先心病发生率排在前五位的省：浙江、广东、陕西、湖南和云南五个省。结论 出生缺陷发生率以先心病最高。

Epidemic trends of birth defects （congenital heart disease）in China and Yunnan from 1986 to 2014
(Abstract) Objective： to understand the monitoring status of congenital heart disease in China and Yunnan from 1986 to 2014 and to provide a scientific basis for the prevention and treatment of the disease.
Methods: the monitoring and annual reports of "National Maternal and Child Health Surveillance Network" and "Three Networks" in Yunnan Province were used. By using SPSS 15.0 software to organize, calculate and chart, the incidence of congenital heart disease in 2014 was systematically clustered and regressed Analysis, and also used PEM8 3.0 software to do the ascending trend test. (Cox stuart test)
Results 1. The incidence of birth defects nationwide increased from 1996 to 2014.
2. From 1989 to 2014, there was a wave decline. The overall trend increased with the passage of time. The incidence of birth defects in Yunnan Province was below the national average。 3. Types of birth defects. The major birth defect nationwide was congenital heart disease from 2005 to 2014, except for the total cleft lip in 2000.
4. The top 5 incidence rate of congenital heart disease nationwide in 2014 are Zhejiang, Guangdong, Shaanxi, Hunan and Yunnan.
Conclusions: the incidence of birth defects is highest in congenital heart disease.
出生缺陷是中国婴儿发病、致残与死亡的主要原因之一，严重影响出生人口素质，《全国出生缺陷监测》被列为国家《七五》攻关课题之一，1986年10月由原华西医科大学牵头在原卫生部领导下，在全国29个省（区市）945所医院即开始了此项工作，云南省于1986年10月组织省市地级35个医院参加。监测对象为住院分娩的围产儿。出生缺陷医院监测初步摸清了全国主要出生缺陷的发生情况及其变化趋势。现对29个省1996-2010年、云南省1986-2011年围产儿中发生出生缺陷患儿的资料进行回顾性分析：了解全国和云南省常见出生缺陷类型及变化趋势，近年来各省市区（29个市省区）先天性心脏病的发生率最高已为第一位病种，云南省先天性心脏病发生率近年也为最高.寻找探讨出生缺陷先天性心脏病发生增高的影响因素，针对影响因素采取相应措施现报导如下：

1、资料来源
1.1.1 全国妇幼卫生监测网[1]、[2]和云南“三网监测”及年报表。
1.1.2 1991-1993年我国南、北方4省30个县（市）的围产保健监测系统与出生缺陷监测系统[3]。
2、方法
1.2.1 《全国29省（市、自治区）出生缺陷监测工作手册》。
1.2.2 统计学分析：用SPSS 15.0软件，整理、运算、制图等，将2014年全国先天性心脏病省市发生率作系统聚类分析和作逐步回归分析，用PEMS 3.0软件作升降趋势检验。

2.1不同年代出生缺陷总发生率
2.1.1 全国发生率：全国出生缺陷发生率149.1/万，出生缺陷发生逐年上升（表1、图1），经升降趋势检验（Cox-stuart），P值<0.01，有显著统计学意义。

2.1.2 云南发生率：云南省出生缺陷发生率118.7/万，1989-2002年出生缺陷发生率呈波浪式下降，2003-2004年有所上升，2005-2007年逐年下降，2008-2010年又直线上升，总体趋势随时间的推移而上升（表2，图2）趋势X2=6.054，P值=0.014，有统计学意义。
